According to the Chest Fever Songfacts guitarist Robbie Robertson, drummer Levon Helm, and pianist Richard Manuel improvised the lyrics over the course of the song. The intro to the song, played on an organ by Band multi-instrumentalist Garth Hudson was improvised as well.
